Medical – $500 deductible, $20 Doctor Co-Pay, Pharmacy & less than $28 per biweekly pay period
Dental – 100% Paid Preventive Care, $50 Deductible, Big Network & less than $3 per biweekly pay period
Vision – $40 Co-Pay for Exams, Annual benefit reimbursement for glasses/contacts
